ENROL Current Students

We the undersigned, certify the applicant is in good health and may participate in activities at Dancing with Maree. In case of emergency requiring medical treatment, the undersigned hereby authorises Dancing with Maree to have the student taken to a medical or hospital facility for treatment and releases Dancing with Maree and its staff from responsibility and indemnity. Dancing with Maree is not responsible for any injuries or accidents that occur before, during or after classes. The undersigned is responsible for their child’s behaviour and safety on the premises. The applicant’s parent/guardian is also responsible for purchasing proper footwear and dancewear necessary for safety and success in class. Dancing with Maree is not responsible for students outside of enrolled class times and parents are required to collect students on time, at the end of their class. The Parent/Guardian is responsible for on time payment of fees; the parent/guardian acknowledges that they have read the Dancing with Maree handbook and are willing to cover costs associated with the end of year concert including costumes and the performers package. The parent/guardian grants Dancing with Maree permission to use the applicant’s name, photograph, or film for publicity purposes.
